The Florida Department of Heath Madison works with community partners to implement evidence-based interventions through the Heart Health Plus Program. Activities including Healthy Heart Ambassadors Blood Pressure Self-Monitoring Program, community health screenings, and process referrals to diabetes, tobacco cessation, and other health and social service programs.

Blood Pressure self-monitoring classes are conducted by a trained Health Educator, and Registered Dietitian. The program offers health education centered around nutrition, lifestyle changes, and blood pressure management.
Participants must enroll and complete the 12-week program. Participants must be clinically diagnosed with hypertension. Participants will receive free self-monitoring blood pressure machine and much more.
